Baptism Registration Form

Greetings in Jesus Christ!

Congratulations!  We look forward to welcoming your child at the celbration of Baptism at the Woburn Catholic Collaborative.  The Baptism will be celebrated at a Sunday ceremony, at the parish of your choice.

If this is your first child, you are required to attend a baptism preparation class prior to the celbration of the baptism.

Families are invited to make a gift to the Lord on the occassion of a child's baptism.  Your gift is voluntary and should reflect your resources.

If you have any questions, please contact the Collaborative Business Office at 781-933-4130.

We look forward to sharing faith and serving you!

Sincerely yours in Christ,

Rev. Thomas J. Powers

Godparents represent the Church. The Church asks that godparents be at least 16 years of age, baptized and confirmed Roman Catholic in good standing and active in their life of faith and worship. If married, godparent's marriage must have been witnessed by Roman Catholic clergy. There can be no more than two godparents, one of each gender. One baptized non-Catholic Christian can witness the baptism in place of a godmother or godfather.
Affirmation and Summary of Intention

We intend to share our Roman Catholic way of life, belief and worship with our child. We attest and confirm that our child's godparents are of age, confirmed and practicing Catholics; if married, done so validly before Roman Catholic clergy and understand their responsibilities and our hopes for our child.
